The Sarah Ralston Foundation (SRF), supports and enhances the quality of life of vulnerable and underserved older adults residing in Philadelphia County. Since our inception in 2023, we have awarded grants to 50 nonprofit organizations that serve older adults—funding innovative projects, and general operating support. But beyond grantmaking, we are equally committed to fostering meaningful connections among our grantee partners.
In this new series, Connecting Nonprofit Partners: Stories of Collaboration, we’re spotlighting the inspiring ways nonprofits are teaming up to expand their reach, share resources, and strengthen support for older adults in Philadelphia.

A unique collaboration between UUH Outreach, Face to Face, URDC, and Germantown Avenue Crisis Ministry is helping older homeowners in Northwest Philly stay safe, stable, and secure in their homes. Through repairs, legal support, and hands-on workshops, Home Strong is changing lives—and preserving intergenerational wealth in communities of color.
When a newly arrived family needed clothing, a simple request sparked a powerful partnership. The Philadelphia Aging People in Prison Human Rights Campaign (Philly APP/HRC) and the African Family Health Organization (AFAHO) came together to meet an urgent need—providing dignity and support through donated clothes. What began with a few bags is now opening the door to deeper collaboration between two mission-driven organizations.
A new partnership between Northwest Mutual Aid Collective and Grand Central Kinship Care is making a meaningful difference for kinship caregiving families in Philadelphia. Through weekly food distributions and direct home deliveries, the two organizations are addressing food insecurity with trust, care, and shared purpose.
